Initial consultation $1950

Introductory price $750. It includes:

• A 60-minute consultation with Dr. Michael
• Evaluation of Functional Medicine Intake forms 
• Functional Medicine Lab tests recommendations
• BIA or Bioelectrical Impedance Analysis specialized test which is an accurate method of measuring 16 markers of health
• Email and text concierge access are included for 90 days from the date of the last visit
• Dr. Michael's new book "Understanding Genomics: How Nutrition, Supplements, and Lifestyle Can Help You Unlock Your Genetic Superpowers"

A $250 deposit is required at the time of scheduling, and a $500 balance is owed before the initial consultation. If you cancel your initial consultation within 7 days of your appointment date, you will forfeit your $250 deposit.

The first follow-up visit is $375. On average, a month after your initial consultation. It includes the following: 

• A 45-minute consultation with Dr. Michael
• Functional Medicine Lab tests Interpretation 
• A Functional Medicine Plan that is personalized to your needs
• A comparative follow-up BIA or Bioelectrical Impedance Analysis specialized test which is an accurate method of measuring 16 markers of health
• Email and text concierge access are included for 90 days from the date of the last visit.
  

Each follow-up visit will cost $275.00. Typically, within three to six months of your previous visit. It includes: 

• A 30 - minute consultation
• Evaluation of treatment plan 
• A comparative follow-up BIA or Bioelectrical Impedance Analysis specialized test, which is an accurate method of measuring 16 markers of health
• Email and text concierge access are included for 90 days from the date of the last visit.

Cancellation policy: 

Treating patients in our practice is one of the most gratifying aspects. It gives us a chance to help people who are looking for answers to their specific health problems and receive heartfelt appreciation from them. Our appointments are longer than those of most doctors. As a result, we provide new patient appointments on at least seven days' notice. 

• If you cancel your initial consultation within 7 days of your appointment date, you will forfeit your $250.00 deposit.
• If you cancel any follow-up appointments less than four days before the appointment, you will be charged the full visit price.
• There will be a 100% office visit charge for “no-shows” or late cancellations. 
• There will be no refund for services rendered.

We understand that life happens and there may be times when you need to reschedule or cancel an appointment. If this is the case, please let us know as soon as possible.

Concierge Medicine:

There is no charge for this service, unlike many concierge physicians. Patients are charged on a per-visit basis. 

Practitioner Grade Supplements and Neutraceuticals:

We offer a variety of supplements and nutraceuticals that are only available through healthcare practitioners. These products are of the highest quality and have been proven to be effective.

Precision Functional Medicine is what we focus on, and each program is designed to meet your individualized specific needs. The recommended Practioner-Grade supplements and Neutraceuticals will be offered as add-ons and priced during your first follow-up visit with us.

Functional Medicine Lab Testing:

Functional Medicine Lab testing is an integral part of our program. These tests help us to identify the root cause of your health concerns and formulate a treatment plan that is tailored to your specific needs.

The cost of these tests will be discussed with you during your initial consultation.

Payment options:

We provide a variety of payment options to ensure that every one may take advantage of our services. We offer interest-free in-house financing and Care Credit. Cash and credit cards are all accepted forms of payment.

Insurance:

We do not accept any insurance. Upon request, we can provide you with the necessary documentation and receipts to submit to your insurance company for possible reimbursement of costs (Medicare/Medicaid excluded). All of our services use standardized insurance billing codes to make this easier. Please keep in mind that we are Medicare opt-out providers, which means we are not part of the Medicare system and do not accept Medicare patients.

Emergency and after-hours care: 

We are a specialty health clinic; therefore, we do not provide primary care. We are only available for consultative services. In the event of an emergency or urgent issue, you must have access to a primary care physician with whom you may discuss things further. If you perceive an adverse reaction to one of the Clinic's treatments, stop using it and notify your primary care physician and us immediately. If you have a serious medical condition that requires immediate care, you should contact your other doctors(s), call 911, or be taken to the closest hospital emergency department.

Treatment plan risks and benefits: 

Each treatment plan and/or process comes with both risks and benefits. If you want further information, please ask questions. Although your treatment regimen will be thoroughly investigated and tailored to your individual personal goals and health conditions, no assurances can be given regarding the results of that treatment plan(s) or procedure(s).

Confidentiality: 

The information that is revealed during office visits is confidential. Unless you give us permission to do so in writing, your record and the data it contains will not be shared with others. Disclosure of a desire to inflict harm on yourself or others, as well as compelled disclosure by specific government agencies under HIPAA rules, are exceptions to this confidentiality.
